Frequently Asked Questions

+ I have a question about my appointment/for my doctor.

The invite to sign up on our platform was sent out by your doctor’s office to request details about your health. For any medical or appointment related questions, please reach out to your doctor’s office directly. The phone number can be found in your invite and reminder emails.


+ My date of birth is incorrect.

We’re sorry to hear that you’re experiencing difficulties verifying your date of birth. The date is entered by your doctor’s office and can sometimes be filled out incorrectly. Please either reach out to your doctor’s office or support@thrive.health with the correct birthdate and we will be happy to assist you.

+ Difficulties logging in.

  1. Please start by clicking the blue Get Started button in the invite email from your doctor’s office. alt text
  2. After clicking on this button, you should be taken to a signup page called Welcome to a Better System. After reviewing the short text, you can continue by pressing the blue Continue button at the bottom of the screen.

  3. You will now be taken to a page called Complete Your Account Details. Since you clicked on the original email invitation, your email address should be pre-filled in the email field. This means you only need to create a password. Your password should contain an Upper case letter, lower case letter, and a number.

  4. Once you have created your password, you can continue to the Privacy and Terms of Use consent box, which must be checked to continue.

  5. Once you’re ready to proceed, click the blue Sign Up button to continue into your questionnaire.

If you’ve followed these steps but are still running into difficulties logging in, please reach out to support@thrive.health and let us know at which step you’re encountering issues. Our support team is happy to help.


+ Reminder emails

The invite to sign up on our platform was sent out by your doctor’s office to request details about your health. Reminder emails are sent out when you have incomplete details on your Thrive Health account. There are a maximum of 5 reminder emails sent out automatically.

If you don’t plan on filling out a questionnaire for your doctor through our platform, please call your doctor’s office and request they delete your invite to our platform.

If you have submitted your questionnaire, and are still receiving reminder emails, your doctor may have requested that you complete multiple questionnaires. Please log in to your Thrive Health account to see if you have any incomplete tasks. If you have any questions about why your doctor requires multiple questionnaires, please reach out to their office directly. The phone number can be found in your invite and reminder emails.


+ I want to update the information in my questionnaire.

The invite to sign up on our platform was sent out by your doctor’s office to request details about your health. To update your questionnaire answers, personal information, or for any corrections, please reach out to your doctor’s office directly. The phone number can be found in your invite and reminder emails.

If you would like to change the answers in your questionnaire, please ask the office for an update request. The medical office assistant will be able to send you a new email enabling you to edit your questionnaire answers. When you have updated your information, please be sure to click the blue submit button.

+ I received an invite email to sign up on Thrive Health’s platform - what does this mean?

Hospitals, doctor’s offices, and health-related organizations use our platform to connect with patients and clients.

Your doctor’s office sent the invite email through our system to request details about your personal health.

+ How can I create an account?

If you received an invite email, just click the button in the email to start the sign up process on our platform. It will only take a minute for us to walk you through the steps.

If you received a registration code from the office, go to app.thrive.health and sign up with your code.

+ Is my personal health information shared with anyone?

Personal health information that you provide though our platform is only shared with your care team, and you can decide who you want to share your information with.

+ Is it safe to complete this questionnaire online?

All data provided or received though our platform is encrypted and your personal information is stored within Canada. We follow data security and privacy standards that align with federal and provincial conformance guidelines such as HIPPA, PIPEDA, PCI, and NIST.

+ How do I manage my account in the long term?

You can log into your account anytime at app.thrive.health and manage your personal information on the home page.
